Information We Collect
This notice applies to all information collected or submitted on the FMS web site. On some pages, you can order products, make requests, and register to receive materials. The types of personal information collected at these pages are:

Credit/debit card information
To gain access to members-only resources and personalization features on the FMS web site, FMS members are asked to provide their login and password. Nonmembers who wish to make a purchase on the FMS web site are asked to create an account.
How We Use Information
FMS will use e-mail addresses for internal use only, and at no time will we sell or rent your e-mail address to a third party. Both members and nonmembers with FMS accounts may manage their communication preferences by logging in and selecting "Update Profile."
FMS makes member contact information available to other members through the FMS Membership and Peer Consulting Directory. If you participate in an educational program, FMS may make your contact information available to other participants and the faculty.
On occasion, FMS may provide mailing information (name, job title, company and business address) to third parties. If you do not wish FMS to release this information, please send a written request to info@fmsinc.org or 100 W. Monroe St., Ste. 810, Chicago, IL 60603.
We use non-identifying and aggregate information to better design our web site - such as the number of visitors per day - but we do not disclose anything that could be used to identify you individually.
Finally, we never share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.
