This policy applies to all information collected or submitted via the FMS web site. On some pages you can order products, register for educational programs, or request information.
To gain access to various resources and personalization features of the FMS web site, FMS members are asked to provide a login and password. Nonmembers who wish to make a purchase on site are asked to create an account. In either case, the personal information collected is typically your:
- Mailing address
- E-mail address
- Phone number
- Fax number
- Credit/debit card information when ordering a product or service
How We Use Information
FMS will use email addresses for internal use only, and at no time will we sell or rent your email address to a third party. Both members and nonmembers with FMS accounts may manage their communication preferences at any time by logging in and selecting "Update Profile."
FMS does make member contact information available to other members through the FMS Membership and Peer Consulting Directory. Whether a member or nonmember, if you participate in an educational program, FMS may make your contact information available to other participants and/or the faculty.
On occasion, FMS may provide postal mailing information only (name, job title, company and business address) to third parties. If you do not wish FMS to release this information, please send a written request to firstname.lastname@example.org or 1 N. LaSalle St., Ste. 875, Chicago, IL 60602
Finally, we never share your personal information provided to us online in any way beyond what is described above without providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.
How You Can Correct Your Information
You can correct errors in your personal information by logging into your account and selecting "Update Profile" or contacting us at 312-578-1300 or email@example.com. To protect your privacy and security, we will take reasonable steps to verify your identity.
